Cause and effect diagram is also called fish bone diagram or Ishikawa diagram. For improvements, sometimes efforts are made to remove the symptoms of the problem rather than the causes. Cause and effect diagram identifies the causes of the problem and based on that improvements can be made. Cause and effect diagram looks like a fish skeleton, with fish head as the major problem, ribs showing the major causes and smaller bones refer to the sub-causes. The steps involved in drawing cause and effect diagram are stating the problem, listing major causes of the problem, listing sub-causes, identifying core causes that are to be eliminated and finally setting goals to address the core causes.
